Education

Dr. Lark is a 1981 graduate of the University of Detroit School of Dentistry and has practiced general dentistry in Toledo, Ohio for the past 37 years. He has been recognized by the American Board of Dental Specialties and Ohio State Board of Dentistry as a specialist in Temporomandibular Joint (TMJ) Disorders, Orofacial Pain, Implantology, and Sleep Medicine. He is one of only five Orofacial Pain specialists in Ohio, serving the Northwest Ohio and Southeastern Michigan areas. Dr. Lark is also InstaRisa trained.

Awards

Dr. Earnest Bach Award (2010): Dr. Lark was honored by his peers with the Dr. Earnest Bach Award for Outstanding Contributions to Dentistry and Service to the Community. The award was established in 1966 by the Toledo Dental Society to honor Dr. Earnest Bach for his leadership in dentistry and community service. Dr. Lark, a past president of the Toledo Dental Society, is the 20th Toledo Dentist to receive this prestigious award.

Dr. Lark has earned Mastership of the Academy of General Dentistry, the highest level of achievement for members in accordance with AGD's mission to advance general dentistry and oral health through quality continuing education.

Dr. Lark earned his Board Certification in the American Academy of Orofacial Pain in 1994 and is also a Fellow in the American Academy of Orofacial Pain and was elected President of this prestigious organization in 2009. The American Board of Orofacial Pain is not a recognized specialty by the American Dental Association or the Ohio State Dental Board; however, The Committee on Dental Accreditation is currently in the process of certifying orofacial pain graduate programs through the ADA.

Dr. Lark was bestowed with a Fellowship in the American College of Dentists and the International College of Dentists.

Dr. Lark has earned his Implant Surgery Certificate from Temple University and the Misch Implant Institute and has been awarded Fellowship status in the International Congress of Oral Implantology.

Dr. Lark has earned his Orofacial Pain certificate from Rutgers University focusing on neurobiology, neurovascular problems, neuropathic headaches, neuroglia problems, temporomandibular disorders, sleep and breathing disorders, snoring, and psychological pain management.

Dr. Lark is a Scientific Advisor at the prestigious Kois Center for Advanced Dental Education in Seattle, Washington. The Kois Center is a didactic and clinical program with the latest advances in esthetics, implant, and restorative dentistry.

Life & Hobbies

Dr. Lark is an avid reader of history, and he enjoys traveling to sites of historic significance. He loves the challenge of crosswords, especially the New York Times's Sunday puzzles. Dr. Lark is a cross-country bicyclist who has completed many century rides (100 miles) and formerly participated in Road Rallies, including his record 425 miles in a twenty-five hour period during his college days. Dr. Lark enjoys playing golf, swimming, and playing bocce ball. Most of all, he treasures quality time with his family. He and his wife of 28 years, Brigid Murphy-Lark, reside in Sylvania, Ohio, with two daughters who are graduates of Ohio State University and the University of Cincinnati. Ariel is a graphic designer who lives in Los Angeles and Meredith is an actress who lives in Queens, New York. His daughter's locations allow Dr. Lark and his wife many opportunities to visit interesting bicoastal areas. Dr. Lark's mentor and father, Lawrence, now lives in Florida. He served as a dentist for 45 years in Ida, Michigan.

Rosemary Chaban, DDS

Education

Dr. Rosemary Chaban graduated with her bachelors from Kent State University in 2013. She went on to obtain her Doctorate of Dentistry from Howard University in 2017 and completed her Residency with the University Of Toledo Medical College the following year. After accepting a position with Oak Openings Dental in 2018, Dr. Chaban has continued her education by working to develop her specialty in orofacial pain, neuropathic pain and TMD treatment. She completed the TMD Oralfacial Pain course at Rutgers University in 2019 and studies with the American Academy of Orofacial Pain annually in addition to her education with the KOIS Institute. Dr. Chaban is InstaRisa trained. She strives for excellence in her career by taking steps to be informed on the latest dental techniques and research by attending local and national symposiums. She helps to promote these best practice guidelines by participating in interdisciplinary cohorts with the area’s top ENTs, orofacial surgeons, and orthodontist.

Life & Hobbies

In Dr. Chaban’s free time she practices yoga, attends local events, participates in fund raisers, and completes DIY projects with her husband.
