My Mission

"To approach each individual patient with care, warmth and empathy. To listen carefully to better attend to their dental concerns. I am dedicated to study and to keep abreast of current science in the delivery of the best dental care available. To provide dental care in a clean, friendly, and professional, safe environment."


Education

Dr. Lark is a 1981 graduate of the University of Detroit School of Dentistry. He has practiced general dentistry in Toledo, Ohio for the past 37 years with an emphasis on Temporomandibular Disorders, Orofacial Pain and Implantology. Dr. Lark is a specialist in Orofacial Pain, Temporomandibular Disorders, and Sleep Medicine, as recognized by the American Board of Dental Specialties and Ohio State Board of Dentistry. He is one of only five Orofacial Pain specialists in Ohio, serving the Northwest Ohio and Southeastern Michigan areas.Dr. Lark also achieved a Masters in both the Academy of General Dentistry and International Congress of Oral Implantologists.


Continuing Education & Professional Memberships

Dr. Lark's qualifications include the following continuing education accomplishments: Master in the Academy of General Dentistry, Diplomate American Board of Orofacial Pain, FICD and FACD, Implant Dentistry Certificate from Temple School of Dentistry, and Orofacial Pain Certificate from Rutgers University.


Life & Hobbies

Dr. Lark is an avid reader of history, and he enjoys traveling to sites with historic designations. He loves being challenged with crosswords, especially the New York Times's Sunday puzzles. Dr. Lark is a cross country bicyclist who has completed many century rides (100 miles) and formerly participated in Road Rallies, including his record 425 miles in a twenty-five hour period during his college days. Dr. Lark enjoys playing golf, swimming, and playing bocce ball. Most of all, he treasures quality time with his family. He and his wife of 28 years, Brigid Murphy-Lark, reside in Sylvania, Ohio, with two daughters who are graduates of Ohio State University and the University of Cincinnati. One daughter, Ariel, is a graphic designer who lives in Los Angeles; the other, Meredith, is an actress who lives in Queens, New York. His daughter's locations allow Dr. Lark and his wife many opportunities to travel to interesting bicoastal areas to visit. Dr. Lark's mentor and father, Lawrence, now lives in Florida. He served as a dentist for 45 years in Ida, Michigan.


Awards

Dr. Earnest Bach Award 2010:

Dr. Lark was honored by his peers with the Dr. Earnest Bach award for outstanding contributions to Dentistry and Service to the Community. The award was established in 1966 to honor Dr. Earnest Bach, who was a leader in Dentistry from the Toledo Dental Society. Dr. Lark, a past president of the Toledo Dental Society, is the 20th Toledo Dentist to receive this prestigious award.

Dr. Lark earned his Board Certification in the American Board of Orofacial Pain in 1994 http://www.abop.net/ and is also a Fellow in the American Academy of Orofacial Pain and has been elected President of this prestigious organization for 2009. http://www.aaop.org/. The American Board of Orofacial Pain is NOT a recognized specialty by the American Dental Association or the Ohio State Dental Board, however The Committee on Dental Accreditation is currently in the process of certifying orofacial pain graduate programs through the ADA which is stages of the process to develop certification as a recognized specialty.

Dr. Lark has earned his Implant Surgery Certificate from Temple University and the Misch Implant Institute and has been awarded Fellowship status in the International congress of Oral Implantology. http://www.icoi.org/

Dr. Lark has earned his Orogacial Pain certificate from Rutgers Univeristy focusing on neurbiology, neurovascular problems, neuropathic headaches, neuroglia problems, temporomandibular disorders, sleep and breathing disorders, snoring, and psychological pain management.

Rosemary Chaban, DDS

Education

Dr. Chaban is home grown from the Toledo area. She graduated from Sylvania Northview High school in 2009. Got her BS degree from Kent State University in 2013. Graduated from Howard University College of Dentistry in 2017. Attended a one year General Practice Residency at the University of Toledo Medical College. She is certified in Invisalign, the wave one system for root canals and implants.



Life & Hobbies

When she isn't practicing, Dr. Chaban enjoys spending time with her husband, family and friends. She is an avid yoga practitioner and might be seen doing handstands around the office, with festive tennis shoes in the air.
